We are looking for a Case Management Supervisor temp to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client s health and human services needed.
Hours: M-F 8am-5pm
$34-$35 Per hour
Responsibilities
Responsible for supervision, performance appraisals, scheduling, coaching, staff development, hiring, training, and disciplinary action of the CCFA Floaters for all offices.
In conjunction with other department supervisors, monitor and audit a sampling of work processed for quality assurance.
Monitoring and approving department floater needs by reviewing requests and strategically allocating resources accordingly.
Responsible for maintaining a knowledge and understanding of Title 5, Funding Terms and Condition, and Agency Policy.
Work with department manager to conduct program evaluation, write progress reports monthly.
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
Handle case assignments, draft service plans, review case progress and determine case closure
Help clients achieve wellness and autonomy
Facilitate multiple care aspects (case coordination, information sharing, etc)
Arrange abeyances with social services, health and governmental agencies
Record cases information, complete accurately all necessary forms and produce statistical reports
Adhere to professional standards as outlined by protocols, rules and regulations
Requirements
Proven working experience in case management, including medical, mental health care manager or a related job
Education Requirement:Associate s Degree in the field of Psychology, Sociology, Child Development, Social Work, Human Services, Family Studies, or related field (as approved) or 2 years equivalent related work experience. Bachelor's degree preferred.
Excellent knowledge of case management principles, healthcare management and reimbursement
Previous experience with psychological aspects of care
Effective communication skills
Excellent organisational and time management skills
Familiarity with professional and technical emerging knowledge
